Purpose:
To objectively quantify the repeatability of intraoperative wavefront aberrometry (IWA) for diagnostic accuracy in refractive cataract surgery
Setting:
Vienna Institute for Research in Ocular Surgery
Methods:
In 50 eyes of patients scheduled for refractive cataract surgery, IWA refraction was performed at 5 defined measurement time points using a 1-piece plate-haptic hydrophilic acrylic IOL. Test-retest repeatability of intraoperative phakic, aphakic and pseudophakic measurements were evaluated. IWA measurements were compared with preoperative objective wavefront measurements for distinction between sources of variability due to inherent intraoperative wavefront quality and sources of variability attributable to the intraoperative setting.
Results:
Out of 38 eyes, 32 had successful consecutive aphakic measurements. Aphakic SE measurements showcased a high consistency with a mean difference of -0.01 diopter (D) and a limits of agreement range of +1.06 (D) and -1.09 (D). A myopic shift of 0.31 sph (D) was observed between preoperative wavefront measurements and phakic IWA. Mean induced astigmatism of 0.30 (D) between preoperative wavefront measurement and aphakic IWA was present. Quantification of RMS error with respect to the Zernike fit affected IWA variability irrespective of variability due to the intraoperative setting.
Conclusions:
Aphakic intraoperative aberrometry was repeatedly feasible, but large limits of agreement due to sources of variability attributable to the intraoperative setting need to be addressed and implemented in frameworks for intraoperative aphakic IOL power selection
