Purpose:
To describe a new technique for transconjunctival intrascleral fixation of Cionni CTR using double flanged polyprolene suture.

Methods:
Cionni ring is inserted in the empty decentered bag after I/A step. A polyprolene 6/0 suture is passed through the eyelet of the ring to be retrived outside the globe via the lumen of 27-gauge needle which is inserted transconjunctivally. Ophthalmic cautery is used to create a flange at the end of prolene suture. Then the prolene suture is pulled till the Cionni ring is centered. The other end of the prolene suture is firmly held with a forceps and maximally pulled through the scleraland conjunctiva then cauterized to make a second flange then released to be under the conjunctiva.
Results:
This technique were used in 7 cases with severe (more than 180 degree) lens subluxation. three cases with hereditary sublyxation (Marfan syndrome), 2 cases were traumatic subluxation and 2 cases with pseudoexfoliation.
All cases achieved a good post operative IOL centration
Conclusions:
Scleral fixation of Cionni ring using a double flanged 6/0 prolene suture is a safe , efficient and time sparing technique
