Purpose:
To assess the surgical and refractive outcomes of a new technique of sutureless scleral fixation (SSF) using a single-piece foldable acrylic Carlevale-lens® (Soleko)
Setting:
The study was conducted at AOUC Careggi hospital in Florence, Italy
Methods:
This is a prospective, observational study of 32 patients with aphakia, dislocated IOL or subluxated crystalline lens who underwent sutureless scleral fixation of a monofocal hydrophobic acrylic Carlevale lens® with handshake technique. Patient’s data including pre-operative and post-operative refractive status, complications during and after surgery were recorded. Intraocular lens tilt was evaluated with anterior segment (AS)- OCT.
Results:
Mean age was 78.25 ± 4.24 years. Twenty-two were IOL/bag luxation, 9 were aphakia and 3 were subluxated crystalline lens. Mean pre-operative BCVA was 0.46 ± 0.29 and at 8 month follow-up improved to 0.13 ± 12 logMAR (p<0.05) with a predictive error (PE) spherical equivalent of -0.24 ± 0.81 D. Intraocular IOL tilt found was 2.08° ± 1,19°. One patient (3.1%) experienced transient CME, and 1 (3.1%) vitreous hemorrhage. Lens dislocation, conjunctival erosion and plugs externalization were not observed in any eye during the follow-up.
Conclusions:
In conclusion SSF using Carlevale Lens® implanted with the handshake technique represents a valid option for the management of aphakia, IOL-bag complex dislocation and crystalline lens subluxation with unique characteristics of stability. The advantages of this technique are the absence of the haptic manipulation, self-centration and the firm fixation of the lens.
