Purpose:
To compare the incidence of post-operative corneal edema between diabetic and non-diabetic patients after phaco-emulsification.
Setting:
Adult ophthalmology department, 20 august hospital, Ibn Rochd university Hospital, Faculty of medicine, Hassan the 2nd university, Casablanca, Morocco
Methods:
A prospective cohort study was conducted between January 1st, 2019 and March 31st, 2019 in adult ophthalmology department including all patients undergoing cataract surgery by phaco-emulsification. Patients suffering from any type of keratopathy or corneal damage before surgery were excluded, proportions of patients presenting postoperative corneal edema at day 1, day 7 and day 30 and its severity were compared between diabetic and non-diabetic patients.
Results:
376 patients filled the inclusion criteria. 22% of patients were diabetic, all type 2 diabetes. There was no significant difference between the 2 groups during the pre-operative assessment. The relative risk of having a early post-operative corneal edema (43% of diabetic patients vs 27% of non-diabetic patients) was estimated to 1.59 (p-value = 0.004) and was estimated to 2.74 (p-value = 0.004) for persistent corneal edema ( 16% vs 6%).
Conclusions:
Diabetes appears to be a significant risk factor both for developing both early post-operative corneal edema and persistent corneal edema, extra-care should be observed for diabetic patients before, during and after surgery.
