How to choose parameters, while planning refractive small incision lenticule extraction (ReLEx SMILE)? The algorithm

Abstract Details
Purpose:
To present an algorithm of choosing surgical parameters, which is important to plan and perform ReLEx Smile and to analyse the results of ReLEx Smile performed while using this algorithm. ReLEx Smile is becoming one of the more popular corneal refractive approaches to correct myopia and myopic astigmatism. There are many parameters, which should be taken into account by the surgeon while planning the procedure. Mistake in planning can cause intraoperative complications and lead to unpredictable result. It is very important especially for the beginning surgeon to have an algorithm how to choose parameters during planning the operation.
Setting:
Laser surgery clinic SPHERE, Moscow, Russia
Methods:
55 myopic patients (109 eyes) 33�Â�±6 y.o. were included in the analysis. All surgeries were planned with our developed step-by-step algorithm and patients were operated using ReLEx Smile on VisuMax femtosecond laser. Refraction in analysed group was: sphere from -10.0 to -1,5 D, astigmatism up to 2 D. BCVA was 0,99�Â�±0,04, pachymetry from 459 to 617 mkm, planned residual stromal thickness from 290 to 387 mkm, cap thickness from 110 to 130 mkm, diameter from 6,2 to 8 mm, average OZ 6,2 mm, lenticular thickness from 53 to 137 mkm. Results were analysed 6 months postop.
Results:
The average SE after the operation was -0,08 D (�Ð�¾�Ñ� -0,75 �Ð�´�Ð�¾ �,5). 79,5% of patients were within �Â�±0,5 D from target refraction, 95% of patients had UCVA 0,8 and higher. Only 1 eye (less than 1%) lost 2 lines of the BCVA, 6% gained 1 and more lines. No significant intraoperative and postoperative complications occurred. No vacuum loss, no lenticular fragmentation, no incomplete lenticule removal. We have got 3,6% cap tears, 1,8% of epithelial ingrowth, interface opacity near the incision in 1,8% of cases.
Conclusions:
Our algorithm of step-by-step planning of the ReLEx Smile parameters allowed us to avoid mistakes and to decrease the number of complications in comparison to already published results at the start of our experience performing this operation. We got predictable refractive results, fast and safe visual recovery in a wide refractive range of the patients.
