Monofocal toric intraocular lens implantation: visual and refractive results

Abstract Details

Purpose:

Report visual and refractive short term results in patients with >1.5D astigmatism after toric monofocal intraocular lens implantation and compare anterior astigmatism values between 2 different topographers and 1 biometer.

Setting:

Private clinical practice in Barcelona, Spain

Methods:

Prospective case series �â�‰�¥1.5D astigmatism patients and toric monofocal intraocular lens implantation (IOL), after clear lens (CL) or cataracts. Pre and post operative data included: Distance uncorrected and best corrected visual acuities (UCVA, BCVA; logMAR scale), subjective sphere, astigmatism & axis, spherical equivalent (SE), mean K, anterior astigmatism and axis with Orbscan (DP-3002 v.3-14.), spot reflector topographer (CASSINI i-Optics), and biometer (IOLMASTER 500, Carl Zeiss Meditec AG, Germany). Safety index (post / preoperative BCVA), efficacy index (postoperative UCVA /preoperative BCVA), and predictability (SE deviation from target) calculated. Data analyzed with t-student and regression analysis for astigmatism measurements (p �â�‰�¤ 0.05).

Results:

9.5�Â�±2.3 months follow-up; 20 eyes underwent PRECIZON IOL implantation for cataracts (15%) or CL (85%). Pre/post data: UCVA 2.0�ï�‚�±0, 11/ 0,24�ï�‚�±0.17 (p<0.01), SE -0,68�ï�‚�±2,8/ -0.14�ï�‚�±0.37D(p=0.42), astigmatism 3.2�ï�‚�±1.3D/ 0,53�ï�‚�±0.70D (p<0.05), BCVA 0.19�ï�‚�±0.21/ 0,10�ï�‚�±0.11 (p<0.05). All improved UCVA, 83% improved final BCVA and no lines lost. Mean lines gained for UCVA 5.5�ï�‚�±1.8 and 2.3�ï�‚�±2.1 for BCVA; 15% arcuate keratotomy. Indexes: Safety: 1.5; Efficacy: 1.5. 95% �Â�±0.50D. SAI vs. preoperative subjective astigmatism R2=0.50 (p<0.05), Orbscan vs Cassini R2=0.64(p=0.34), Cassini vs. IOLMASTER R2=0.72 (p<0.01),

Conclusions:

Toric monofocal IOL implantation is safe, effective, and predictable. It significantly improved VA and reduced subjective astigmatism. Cassini and IOLMASTER astigmatism values strongly correlated.
