Visual outcomes of four presbyopia-correcting intraocular lenses in various configurations

Abstract Details

Purpose:

To compare the visual and refractive outcomes, patient satisfaction and spectacle independence between four novel designs of diffractive IOLs.

Setting:

Visum Clinic, Rzesz�Ã�³w,Poland, Department of Ophtalmology Medical University of Silesia , Katowice, Poland

Methods:

Retrospective study including 83 patients with implantation of four types of IOLs: bifocal diffractive low-add lenses, Tecnis MF (n=20) in mix and match setup, trifocal diffractive IOL Acriva Tri-ED (n=20), trifocal diffractive IOL PanOptix (n = 20), and Symfony IOL implanted in micro-monovision setup (n=23). Patients were followed up on both 3 and 6 months after surgery. The major parameters were monocular and binocular uncorrected (UDVA) and corrected (CDVA) distance visual acuity, uncorrected (UIVA) intermediate (70cm), and uncorrected near (40cm) visual acuities (UNVA), contrast sensitivity (CS), defocus curve, as well as patient satisfaction and spectacle dependence.

Results:

Mean UCDVA was 0,07;0,05;0,01;0,03,UCIVA= J1;J2,83;J2;J2,5, UNVA= J1 J1,8, J1,3, J1 for Symfony, Acriva, Tecnis,Panoptix respectively. Postoperative UNVA was J1 with the Tecnis MF, J1,8 with the Acriva, J1,3 with the Symfony, and J1 with the PanOptix.The Symfony defocus curve showed the smoothest profile. Mean mesopic value of CS was 1,15,1,15,1,07,1,02log units with Symfony, PanOptix ,Tecnis Acriva Tri-ED. 45% patients with Tecnis, 40% with Symfony , 33% with Acriva ,40% with PanOptix reported halos and glare. No reading glasses were used by all patients with PanOptix; 28% with Acriva; 20% with Tecnis MF; Symfony needed glasses only for near distance.

Conclusions:

All four IOLs produce excellent results with high level of spectacle independence and high level of satisfaction. The Symfony produces better levels of distance and intermediate visual acuity, whereas the trifocal IOLs and bifocals produced better near vision. Higher frequency of dysphotopsia was seen with the bi- and Acriva Tri-ED �â�€�“ as well as in Symfony IOLs, but all in mild intensity. No trouble some and disabling photopic phenomena were noticed. Better contrast sensitivity in photo- and mesopic condition were noticed in patients with Tecnis Symfony and Acrysof PanOptix.
