Comparative analysis of visual outcomes and patient satisfaction after bilateral implantation of two different diffractive multifocal intraocular lenses in cataract patients

Abstract Details

Purpose:

To evaluate and compare the performance of the Tecnis MIOL and the Diff-aA MIOL in terms of visual acuity spectacle independence and patient satisfaction in cataract patients.

Setting:

Department of Ophthalmology, Hietzing Hospital, Vienna, AustriaKarl Landsteiner Institute of Process Optimization and QM in Cataract Surgery, Vienna, Austria

Methods:

In this prospective, randomized comparative study 50 eyes of 25 subjects with senile cataract were implanted with the TECNIS MIOL ZLB00 (Abbot Medical Optics Inc.) and 50 eyes of 25 age matched subjects with the DIFFRACTIVA Diff-aA MIOL (HumanOptics). Observation Procedure was performed 1, 3, 6 months postoperatively. Main Outcome Measures included uncorrected distance visual acuity (UDVA) and best-corrected distance visual acuity (CDVA), distance-corrected near visual acuity (DCNVA), distance-corrected intermediate visual acuity (DCIVA) defocus curve, manifest refraction, and a patient satisfaction questionnaire.

Results:

Postoperative UDVA was 0,05�Â�±0,07 and 0,09�Â�±0,09 logMAR (p=0,03); postoperative CDVA was 0,02�Â�±0,05 and 0,04�Â�±0,17 logMAR (p=0,13); DCIVA was 0,16�Â�±0,1 and 0,11�Â�±0,12 logMAR in the ZLB00 and Diff-aA groups (p=0,07); and DCNVA was 0,11�Â�±0,1 and 0,11�Â�±0,1 logMAR in the ZLB00 and Diff-aA groups (p=0,86). Postoperative spherical equivalent was -0,16�Â�±0,25 and 0,14�Â�±0,47 Diopter (p=0,001); Prediction Error was -0,51�Â�±0,48 and 0,09�Â�±0,62 Diopter in the ZLB00 and Diff-aA groups (p=0,001). Defocus curves and patient satisfaction questionnaires suggested similar performance between the groups.

Conclusions:

The Tecnis ZLB00 IOL provided better uncorrected distance visual acuity than the Diff-aA MIOL, which might be explained by the lower predictive error in the former group. Both lenses showed a similar behavior for short and intermediate vision,
