Residual viscoelastic material in the bag, an important factor yielding refractive shifts after plate haptic multifocal IOL implantation

Abstract Details

Purpose:

To evaluate the effects of residual viscoelastic material in the bag upon refractive outcomes after plate haptic multifocal IOL implantation.

Setting:

Veni Vidi Eye Hospital, Istanbul

Methods:

The status of 64 years old hyperopic female patient was evaluated who had bilateral multifocal plate haptic IOL implantations after uneventful phaco surgeries with an unexpected myopic shift occurred in the OS. The uncorrected distant and near visual acuities (UCDVA, UCNVA) of the post-operative era and the final refractive status were recorded. The effective IOL position of the early post-operative period was compared with the post-removal status of the residual viscoelastic material.

Results:

The early post-operative refractions were -0.25 D, -0.50 D at 20 degrees in OD and -2.75 D, -0.25 D at 141 degrees in OS. UCDVA were OD 1.0 and OS 0.2, BCDVA OD 1.0 and OS 1.0. OS anterior segment was shallow on the first post-operative day (2.22 mms). With removing viscoelastic material, the effective lens position changed gradually and the anterior chamber deepened (3.58 mms). The final UCDVA and UCNVA were OD 1.0 and OS 1.0 and the final refractive status were -0.50 D in OD and -0.25 D, -0.25 D at 105 degree in OS.

Conclusions:

Residual viscoelastic material in the bag could be an important factor yielding unexpected refractive shifts especially after plate haptic IOL implantations. Early management of the situation offers good refractive outcomes correcting the effective IOL position in the bag.
