Contrast vision of multifocal intraocular lenses using hybrid (diffractive-refractive) optics with different near additions

Abstract Details
Purpose:
To evaluate contrast vision of hybrid (refractive-diffractive) multifocal intraocular lenses (MIOL) with different near additions.
Setting:
In this prospective interventional clinical trial 60 eyes (30 patients) underwent cataract surgery with MIOL implantation in order to restore near and distance vision. Exclusion criteria were, among others, vision-limiting comorbidities and preceded intraocular surgery.
Methods:
Three apodized bifocal hybrid IOLs with different near additions were compared: �.50 diopters (D) (SV25-type ReSTOR, Alcon; 15 eyes), �.00 D (SN6A-type ReSTOR, Alcon; 15 eyes) and �.50 D (Diff-aAY, HumanOptics; 30 eyes). Subjective refraction, monocular uncorrected visual acuity (VA) and monocular distance-corrected visual acuity were assessed in the far, intermediate and near, under mesopic conditions with high- and low-contrast, and glare. Contrast sensitivities (CS) were evaluated under mesopic and scotopic conditions, and glare. Examinations were accomplished preoperatively; and 1 day, 1 month and 3 months postoperative between March 2013 and May 2017.
Results:
All groups revealed uncorrected distance VAs of 20/20 Snellen equivalents, or better. Distinctions were revealed for corrected distance VAs; hybrid optics with a lower addition were superior to those with higher additions. Vice versa, intermediate and near VAs were superior in MIOLs with higher additions. Regarding high- and low-contrast mesopic VAs, differences between the groups were insignificant. The investigation of mesopic and scotopic CSs; and of mesopic CS with glare revealed an impact of the present near additions; best results were obtained with lower additions.
Conclusions:
Apodized hybrid MIOL with different near additions provide excellent distance and near vision. Yet, the choice of the specific near addition may determine to attain a high CS and distance VA, but slightly reduced near VA; or a high near and distance VAs, but slightly reduced CS.
