Comparative prospective study of intraocular multifocal lenses in refractive lens exchange surgery: FineVision® versus Bi-Flex M677MY® – results at 3 months

Abstract Details

Purpose:

To study the visual outcomes in far, middle and near vision of patients undergoing refractive lens exchange surgery with these multifocal lens. As secondary objectives, complications, refractive results, visual quality, vision-related quality of life and patient satisfaction are assessed.

Setting:

Institut Català de Retina. Barcelona, Spain.

Methods:

Blind prospective observational cohort study. To determine the type of intraocular lens to be implanted, the one that provides a final spherical equivalent value closer to emmetropia has been choosen.Inclusion criteria: bilateral surgery, age ≥45 years, corneal astigmatism ≤ 1 diopter, visual acuity with correction ≤0.10 LogMAR and ≥ J2 (Jaeger scale for near visión), regular topography, hypermetropia ≥ + 1D and presbyopia associated.Exclusion criteria: intraoperative complication that prevents the implantation of the assigned lens, evidence of current or past ocular pathology that prevents the surgery, presence of cataract or other opacity in the lens or previous ophthalmic surgery.Exclusion criteria: intraoperative complication that prevents the implantation of the assigned lens, evidence of current or past ocular pathology that prevents the surgery, presence of cataract or other opacity in the lens or previous ophthalmic surgery.

Results:

A total of 22 eyes corresponding to 11 patients have completed the follow-up at 3 months. 12 eyes were assigned to Finevision group and 10 to the Bi-flex group. The mean age was 54.09 years old.The efficiency index (postoperative UCVA / preoperative BCVA) in the Finevision group reached values of 111,53% (DS:51,21) for far vision; 108,87% (DS:83,29) for middle vision; and 84,13% (DS:32,12) for near vision. The efficiency index in the Bi-flex group obtained values of 85,16% (DS:29,92) for far vision; 80,61% (DS:43,46) for middle vision and 91,64%(DS:26,43) for near vision. No statistically significant differences were found between groups.

Conclusions:

Both lenses provide good refractive and functional results, with a statistically significant improvement in vision-related quality of life (VF14). All the patients had a high satisfaction with the surgery, and 100% would return to intervene with the same technique. The main complaint in both groups has been the presence of halos, being for most patients a moderate problem. A statistically significant change was observed in the pupillary size in mesopic and scotopic conditions.At the level of the defocus curve, for the intermediate vision, the Finevision lens presents a more stable plateau image.
