Comparison of postoperative results after implantation of two preloaded hydrophilic acrylic intraocular lenses (IOLs)

Abstract Details

Purpose:

To evaluate and compare the predictability of intraocular lens power calculation of two preloaded hydrophilic acrylic IOLs with different haptic design.

Setting:

Semmelweis University, Department of Ophthalmology, Budapest, Hungary

Methods:

In this prospective randomised study cataract surgery was performed on 265 eyes using phacoemulsification technique. In Group A (n= 65), Medicontur Z-flex690P IOL, and in Group B (n= 200) Medicontur Bi-flex677P IOL was implanted in the capsular bag. Laser interference reflectometry (Lenstar) and multi formule was used for IOL calculation. At the end of a 3-month follow-up, the difference of manifest refraction and predicted postoperative spherical equivalent (SE) values were compared to evaluate the predictive power of calculation methods (Holladay 1, SRK/T, Hoffer Q, Haigis) and the multi formule (mean-, median error and absolute error (ME, MedE, MAE, MedAE)).

Results:

In Group A, the absolute error (AE, difference of manifest and planned SE) was under 0.5D in 76.7%, and was less than 1.0D in 97.7% of the cases. The results in Group B were 83.8% and 97.3% respectively. Mean error (ME) was found to be significantly higher in Group A (A: -0.29�Â�±0.42 D; B: -0.09�Â�±0.45 D, Mann-Whitney, p<0.05). No significant difference was detected in the average absolute error (MAE, A: 0.40�Â�±0.32 D; B: 0.32�Â�±0.32 D, Mann-Whitney), although the median AE was found to be lower in Group B (A: 0.38 D; B: 0.25 D).

Conclusions:

Postoperative refraction can be planned with high accuracy in the case of implantation of both IOL types. The optic design of the two IOLs are similar, so the possible reason of the slight benefit detected in favor of the Bi-flex 677P can be explained with the difference of haptics. The higher mean error in Group A suggests, that the IOL constants of Z-flex 690P need to be corrected.
