IC-8: Small aperture IOL – the IOL of choice for high corneal irregular astigmatism

Abstract Details

Purpose:

To evaluate the post-operative performance of eyes with high irregular corneal astigmatism implanted with this type of IOL

Setting:

San Bassiano Hospital, Bassano del Grappa (VI), Italy

Methods:

We implanted 3 patients affected by high irregular corneal astigmatism, first patient, aged 45, suffered from monolateral PRK decentration with high irregular astigmatism and high order corneal aberrations, second patient, aged 65, was operated by PKP with high irregular astigmatism in one eye, the third one, aged 60, was operated by hyperopic decentered LASIK in one eye. Pre operative UCDVA and CDVA were performed. UCDVA, UCNVA, CDVA and CNVA were evaluated by ETDRS test after 1st week, 1st month, 3rd months, 6th months. IOL calculation was obtained by IOL Master and SRKT formula, considering 3 mm central K reading.

Results:

All of 3 patients were extremely satisfied after 6 months, mean satisfaction rate grading from 1(poor) to 5 (high) was 4,56 ± 0,23 . The mean pre operative UCDVA was 0,78 ±0,09, CDVA was 0,69±0,09 LogMAR. Post operative UCDVA was 0,16±0,05 LogMAR, CDVA was 0,09±0,90 LogMAR, UCNVA was 0,33±0,05 LogMAR and CNVA was 0,23±0,05 LogMAR. Results remain constant after 6th months.

Conclusions:

All patients increased uncorrected visual acuity for far, and near vision with no glare. All reported “satisfied” or “very satisfied” on overall vision. IOL calculation was difficult due to the uncommon corneal morphology , but pin hole forgive a large amount of post operative IOL power errors.This IOL should be a good solution for complicated cornea with high irregular astigmatism.
