Comparison of visual outcomes after implantation of extended-range-of-vision and trifocal intraocular lenses

Abstract Details
Purpose:
To compare visual outcomes after refractive lens exchange followed by bilateral implantation of two intraocular lenses (IOLs): one extended range of vision IOL, the Tecnis Symfony, and one trifocal IOL, the Acrysof IQ Panoptix.
Setting:
Department of Ophthalmology of a tertiary referral center in Oporto, Portugal - Centro Hospitalar e Universit�Ã�¡rio do Porto.
Methods:
Each group of this prospective comparative study included 30 eyes (15 patients). Refractive lens exchange followed by bilateral implantation of TecnisS�Â�® Symfony IOL (group 1) or AcrySof�Â�® IQ PanOptix�â��¢ (group 2) was performed. Patients were evaluated preoperatively, 1, 3 and 6 months postoperative with macular and retinal nerve fiber layer (RNFL) measurements by Spectral Domain OCT; endothelial cell counts; contrast sensitivity in mesopic and photopic conditions and glare sensitivity at 1, 5 and 100 cd/m2 by Metrovision-MonPack3�Â�®. Best corrected and uncorrected visual acuity (BCVA and UCVA, respectively) for distance, intermediate and near were evaluated. Biometry was assessed using IOLMaster 700�Â�®.
Results:
There were no preoperative differences between groups in all the parameters evaluated. Better UCVA for distance, intermediate and near vision were achieved in both groups (p<0.05). Only the difference in intermediate visual acuity between the 2 groups was significant (p<0.05). Postoperative spherical equivalent within �Â�±1.00D and binocular UCVA for distance of at least 20/25 (Snellen Scale) were observed in all eyes of both groups. No eye developed cystoid macular edema after surgery. The results to date suggest no significant variation and differences between IOLs in postoperative glare and contrast sensitivity, either in photopic and scotopic conditions (both p>0.05).
Conclusions:
Refractive lens exchange with Tecnis�Â�® Symfony or AcrySof�Â�® IQ PanOptix�â��¢ intraocular lens implantation provided similar and good visual function and contrast sensitivity under photopic and mesopic conditions with low perception of halos by patients. Both IOLs provided excellent distance and near visual outcomes however a better intermediate visual outcome was achieved with AcrySof�Â�® IQ PanOptix. The two IOL�â��s had predictable refractive results and optical performance with all patients achieved spectacle independence for far, intermediate and near. A high degree of satisfaction with few complications was obtained with both IOL�â��s.
