Evaluation of objective and subjective visual outcomes of extended-depth-of-focus posterior chamber intraocular lens implantation by micro-monovision approach to correct presbyopia after phaco surgery

Abstract Details

Purpose:

Extended depth of focus (EDOF) intraocular lenses (IOL) are the recent addition to armamentarium of multifocal intraocular lenses (MFIOL) for correcting presbyopia. The present study was conceived to evaluate the objective visual outcomes of this new IOL after bilateral implantation of new Technis Symfony Extended Range of Vision Intraocular Lens (IOL), Model ZXR00 (AMO) with micro-mono-vision approach. Secondary objective of the study was to evaluate subjective outcomes with the help of a questionnaire to assess spectacle independence, satisfaction levels and difficulty level in doing routine daily tasks.

Setting:

It was a prospective single center, descriptive study conducted at Advanced Eye Centre, PGIMER, Chandigarh, India between January 2016 to December 2016

Methods:

Visual outcomes were evaluated in 12 patients (24 eyes) with bilateral implantation of Technis Symfony IOL. After baseline visual-acuity and routine ophthalmological examination, ocular dominance was determined by lens fogging technique. Emmetropia was targeted in the distance dominant eye and myopia of 0.50D-0.75D was targeted in the non-dominant eye. Post-operative examination was performed for each eye on day-1 and day-3 & binocular exams followed 1 and 3 months after second eye surgery. Binocular subjective refraction, Defocus Curve (DC) and Contrast Sensitivity (CS) were evaluated. Patients were asked to fill quality of life questionnaire at the end of 3 months.

Conclusions:

Bilateral implantation of Technis Symfony extended depth of focus IOL with a micro-monovision approach provides excellent visual outcomes with high level of spectacle independence for the patients. Patients are highly satisfied with their quality of vision and report minimal difficulty in performing their daily tasks without spectacles. We would like to propose mini-monovision approach for patients undergoing implantation of EDOF lenses and demanding good visual acuity at near, intermediate and distance rather than emmetropia as a target in both eyes.
