Optical principles and light theories of the actual EDOF IOL for presbyopia correction

Abstract Details

Purpose:

This study is to evaluate the optical differences of the new EDOF IOL in transmitting light. In these lenses the extension of the depth of focus is obtained with different optical principles and different light optical physics theory. The classic paradigms of light dispersion by diffractive plate, and aberrations as enemies of vision, are now replaced by an advanced optical technology able to improve vision performance and correct presbyopia. The last approach of optical engineering is an EDOF IOL with an interferometric phase plate to optimized the light energy distribution.

Methods:

Three different optical principles were considered in the study: 1) Diffractive �â�€�œechelette�â�€� (EROV IOL, Tecnis�Â�® Symfony, ABBOTT, Usa), 2) Modulation of spherical aberration (EDOF IOL, MINI WELL Ready, SIFI MEDTECH, Italy) and 3) Interferometric plate (Instant Focus�Â�®, SAV-IOL SA, Switzerland). The optical features of the three distinctive EDOF IOL surfaces were described and tested using the Zernike polynomials wavefront analysis and ray-tracing simulations.

Results:

The Tecnis�Â�® Symfony (ABBOTT) uses a proprietary achromatic diffractive �â�€�œechelette�â�€� that corrects the corneal chromatic aberration for enhanced contrast sensitivity and generates an extended range of vision. The MINI WELL�Â�® Ready (SIFI MEDTECH) uses spherical aberrations manipulation to obtain a dispersion of focus designed to realize an extended depth of focus able to correct presbyopia. The Instant Focus�Â�® (SAV-IOL SA) is based on the concept to extend depth of focus through the creation of a pseudo non-diffractive beam (PNDB), involving the creation of an optical effect through light projection. Images are projected in the form of a luminous tube.

Conclusions:

The different EDOF IOL tested are optically capable to extend the focus with high optical performance and negligible loss of light energy. Unlike traditional multifocal IOL, the EDOF IOL do not create simultaneous vision with multiple overlapped retinal images, and patient can benefit of a faster adaptation also due to the absence of optical adverse effect and dysphotopsia.
