Phakic intraocular lens explantation: causes and outcome

Abstract Details
Purpose:
To describe the causes for explantation of phakic intraocular lenses (pIOLs) and the outcome of the surgical interventions.
Setting:
Centro Hospitalar de Lisboa Central, Lisbon, Portugal.
Methods:
This retrospective study comprised 13 eyes of 11 patients who were submitted to pIOL explantation between January 2015 and January 2017 in one institution.
Results:
Mean age at explantation was 54.31�Â�±12.78 years (range: 35-78). Mean time between pIOL implantation and explantation was 102.31�Â�±47.21 months (range: 3-140). The main causes of explantation were progressive endothelial cell loss (7 eyes, 53.8%), corneal decompensation (3 eyes, 23.1%) and cataract formation (3 eyes, 23.1%). According to the cause of explantation, treatment was simple explantation of the pIOL (5 eyes), explantation followed by phacoemulsification and in-the-bag IOL placement (5 eyes), concomitant explantation and endothelial keratoplasty (2 eyes) and concomitant explantation and penetrating keratoplasty (1 eye). Mean logMAR best corrected visual acuity (BCVA) improved significantly from 0.50 to 0.14(p=0.001).
Conclusions:
The most frequent reason for pIOL explantation was progressive endothelial cell loss. Simple pIOL explantation, bilensectomy and concomitant endothelial keratoplasty were effective in improving BCVA, according to the causes of explantation.
