Disability due to toxic damage of the optic nerve at alcohol intoxication

Abstract Details

Purpose:

This article describes a clinical case of organ damage as a result of alcohol intoxication. Dana theoretical basis of this disease: the concept of statistical data, the causes are described stage lesions of the optic nerve,the pathogenesis of the disease; the results of examination of the patient: medical history (including history of alcohol), somatic and neurologic, ophthalmologic status,the results of instrumental examination of the dynamics are given activities to the patient's treatment, rehabilitation and employment forecast is presented. Highlighted individual approach in the treatment of this disease in people of different social groups.The analysis of the clinical case,proved its relevance.

Setting:

Federal State Budgetary Institution 'Federal Bureau of Medical and Social Expertise' Ministry of Labor and Social Protection of the Russian Federation (FBMSE)

Methods:

Patient K.,45 y.o.,entered the ophthalmological department of the FBMSE with complaints about acute decrease of the vision of both eyes ,loss of central fields of vision,headache that arose after consuming alcoholic beverages for 1 week.Ophthalmic status:Both eyes-visual acuity of both eyes.Visus OU-pr. l. certa,not corrected.Intraocular pressure is 17 mm Hg.Eyelids without visible pathology,conjunctiva not changed;Cornea is transparent;Anterior chamber of medium depth,uniform;moisture of the anterior chamber is clean;Pupil of regular shape;Crystalline lens and vitreous body are transparent.Eye fundus:diffusive edema in the macular region,retinal hemorrhages in the papillomacic bundle,pronounced edema;disc of the optic nerve is pale,the boundaries are clear,excavation is physiological.

Results:

Complex treatment,including detoxification,metabolic,angioprotective,anti-inflammatory therapy,vitamin therapy,antioxidants was performed on the patient.As a result of the treatment,the visual functions of the patient on both eyes improved within 2 weeks.Visual acuity OD= 0.2 n / c, OS= 0.3 n / c; Intraocular pressure OD:16 mm Hg; OS:16 mmHg. Data of optical coherence tomography showed a positive dynamics,a decrease in edema in the central parts of the retina.According to computer perimetry, restoration of the central sections of the visual fields was detected.Due to the presence of persistent moderate sensory disorders,patient is diagnosed with a third disability group for a period of one year.

Conclusions:

Presented clinical cause is interesting in that this pathology is widespread worldwide,but is not fully studied.Toxic lesion of the optic nerve is one of the causes of vision impairment.Pathogenesis,early diagnosis and treatment of this disorder represent an important problem of modern ophthalmology.This is due to the severity of lesions,an unfavorable prognosis,an increase in the frequency of toxic diseases of the optic nerve.Analyzing observation above,we can summarize following outcome:-early diagnosis of damage to the organs of vision,optic nerve and provision of timely treatment;-in the presented clinical observation results of treatment,confirm the effectiveness of the attendance of the optical nerve.
