Femtosecond laser versus mechanical keratome LASIK for myopia in 49,263 eyes of 27,343 patients at a single institution in Asia

Abstract Details

Purpose:

To examine differences in efficacy, predictability, safety and complications between femtosecond and mechanical microkeratome laser in situ keratomileusis (LASIK) for myopia at a single institution in Singapore from 1998 to 2015.

Setting:

Singapore National Eye Centre

Methods:

Prospective, nonrandomized, single-center study of all patients who underwent myopic LASIK surgery at the Singapore National Eye Centre from 1998 to 2015. Pre- and postoperative refractions, uncorrected visual acuity, and best-corrected visual acuity were documented. Outcome measures of safety, efficacy, refractive predictability and complications were analysed for mild, moderate, and high myopia according to spherical equivalence (SE) of less than 5.00 diopters (D), 5.00 D or more to less than 10.0 D, and 10.00 D or more, respectively. MK and FS LASIK, in terms of efficacy and safety index was compared by adjusted R-squared regression analysis.

Results:

There are 14307 eyes in the femtosecond (FS) laser and 38128 eyes in the mechanical keratome (MK) lasik group. The FS group showed better efficacy index (FS 0.96, MK 0.90) and refractive predictability, FS group had 96% of eyes within �Â�± 1.0D and 74% within �Â�± 0.5D compared to MK group with 91% of eyes within �Â�± 1.0D and 66% within �Â�± 0.5D. Adjusted R-squared regression analysis suggests either MK or FS LASIK does not affect the efficacy index. However safety index is higher in FS LASIK group. Incidence of flap related complications is lower in the FS group.

Conclusions:

FS LASIK has an advantage in safety and predictability but not efficacy measures when compared with MK LASIK.
