Prevalence of keratoconus and subclinical keratoconus in young men with refractive error using Scheimpflug tomography parameters

Purpose:
To determine the prevalence of keratoconus (KCN) and subclinical KCN among young men with refractive errors, and to compare Sheimpflug Tomography parameters among these subjects.
Setting:
National Ophthalmology Center named after acad. Z.Aliyeva, Azerbaijan.
Methods:
A total of 11670 young men with different refractive errors were included in the study from January 2015 to January 2017. All subjects underwent a complete ophthalmic examination which included refraction, visual acuity measurement, slit lamp biomicroscopy, retinoscopy, fundus examination and Sheimpflug corneal tomography with Pentacam.The diagnosis of KCN and subclinical KCN was made by observing clinical findings and topographic features; and confirmed by corneal thickness and elevation maps of Sheimplug tomography. Several parameters acquired from Pentacam were analyzed employing the Mann-Whitney U Test.
Results:
Mean age of the study population was 21.3�Â�± 2.7 (range 18-34) years. From the total number of subjects 322 (96.1 %) had bilateral keratoconus and 13 (3.9%) were diagnosed as unilateral keratoconus. Among keratoconus patients 30% were assessed as subclinical KCN, 28% as mild, 28.3 % as modarete and 13.8% as severe KCN. There was no statistically significant correlation between age and disease severity. Maximum corneal power, index of vertical asymmetry, keratoconus index, elevations, overall D values were significantly higher and pachymetry was significantly thinner in eyes with clinical or subclinical KCN than topographically normal eyes (P< 0.05).
Conclusions:
Early keratoconus detection very important in young males not only for the early treatment, also for the assessing to pass military servis. The current study showed that for the early detection of keratoconus, mens with suspect on keratoectasia should undergo corneal topography screening in preliminary examination for military service. Sheimphlug tomography may provide more accurate information about anterior and posterior corneal anatomy especially in suspect eyes. The prevalence of keratoconus among young men population was closer to the results obtained in other population-based studies comprising both men and women in the same age group.
