HD-OCT for imaging anterior chamber inflammatory reaction after cataract surgery: clinical correlation and grading

Purpose:
To evaluate the anterior chamber inflammatory reaction by anterior segment HD-OCT.
Setting:
Hospital Universitario Fundacion Alcorcon, Madrid, Spain.
Methods:
A prospective, nonrandomized, observational case series. 30 eyes of 30 patients were studied for AC reaction clinically and by anterior segment OCT measured at the center of the anterior chamber. Hyperreflective spots suggesting the presence of cells in the AC from the OCT images were counted manually and by a custom made automated software. Correlation with clinical grading of AC cells using Standardization of Uveitis Nomenclature criteria was used.
Results:
Significant correlation was seen (Cohen`s kappa= 0.5) in mean values between the manual and automated method. AC cells were detected in 24 eyes (80%), with corneal edema with central corneal thickness ranging from 502 to 784 microns. Out of 30 eyes, grade 4 aqueous flare was detected by OCT imaging in 4 eyes and clinically in 2 eyes.
Conclusions:
Anterior segment HD-OCT can be used as an imaging modality in detecting AC inflammatory reaction post phacoemulsification, even in eyes with decreased corneal clarity and compromised AC visualization attributable to corneal edema. The higher the inflammation, the more sensitive is the authomated method.
