Comparative study of refractory glaucoma surgery by using different shunts

Purpose:
To study clinical results of refractory glaucoma surgery by using Leucosapphire Drainage Device and EX-PRESS Glaucoma Filtration Device (GFD) and to compare the efficacy and safety between the Leucosapphire and the EX-PRESS GFD shunt implantation
Setting:
St.Petersburg Branch of S.N. Fyodorov IR&TC ��Eye Microsurgery��, Russia
Methods:
In our prospective study we included 60 patients (60 eyes) with refractory open-angle glaucoma. In all cases implantations were performed after prior unsuccessful operations without surgical complications. Two types of devices were used for implantation. The Drainage Device is made of leucosapphire with 300 m�Â�µ internal lumen size unlike EX-PRESS GFD of stainless steel with 50/200 m�Â�µ internal lumen size. 30 patients (50%) underwent Leucosapphire Drainage implantation and 30 patients (50%) underwent EX-PRESS GFD implantation. Surgical details, intraoperative and postoperative complications, visual functions, intraocular pressure (IOP), optical coherence tomography (OCT) examination, number of used medications were evaluated.
Results:
During surgery complications were not observed. We recorded such serious complications as hyphema and choroidal haemorrhage in 16.7% cases (10 eyes). In 6 -18 months after operation good IOP control occurred in 85% eyes. Success rates were 86.7% (26 eyes) for Leucosapphire Drainage implantation and 83.3% (25 eyes) for EX-PRESS GFD implantation. Rates of failure and hypotony were not significantly different between the groups. There was no significant difference in IOP reduction and a number of used postoperative IOP-lowering medications between these procedures.
Conclusions:
Our study shows that Leucosapphire Drainage and EX-PRESS GFD implantation in eyes with refractory glaucoma is safe and efficacious. There was no significant difference between the Leucosapphire Drainage and EX-PRESS GFD implantation.
