Comparative clinical results of the 250 vs 350 Baerveldt glaucoma drainage implants in refractory glaucoma

Abstract Details

Purpose:

To compare the short term clinical outcomes of a large surface area Baerveldt 350 mm2 glaucoma drainage device (GDD) with medium surface area Baerveldt 250mm2. Design: Retrospective non randomized comparative trial

Setting:

Research Department, Oftalmosalud Eye Institute, Lima-Peru, Av. Javier Prado Este 1142, San Isidro, Lima, Peru

Methods:

A total of 35 eyes of 35 patients with mixed glaucoma diagnoses who had undergone a glaucoma drainage device surgery in a period performed between 2015 and 2016, 19 with eyes either a large Baerveldt 350mm2 GDD and 16 eyes with a medium Baerveldt 250mm2 in Oftalmoslud Institute Per�Ã�º were reviewed. The primary outcome measure was surgical success. Succes was defined as IOP <18 mmHg with or without drugs and without additional glaucoma procedures during the time of the study. Secondary outcome measures included intraocular pressure (IOP), best corrected visual acuity (BCVA) and number of medications. IOP, BCVA were measured preoperatively and at 1 and 6 months. Number of drugs was measured preoperatively and at 6 months. Complications were noted at every control visit.

Results:

The Baerveldt 350 mm, the mean�Â�±SD preoperative corrected visual acuity to 1,3,6 month were 0.94, 0.17, 0.13 There was no significant difference. The preoperative intraocular pressure obtained from preoperative to 1,3,6 month follow-up we found significant difference on 1,3, 6 month ( P=0,00),(P=0,00),(P=0,00) In the antiglaucomatous treatment the mean�Â�±SD preoperative up to 6 month was 3.55�Â�±0.87 to 2.55�Â�±0.94. Complete success rate was 21% and qualified success of 94.7%. In the Baerveldt 250mm, the mean�Â�±SD preoperative corrected visual acuity to 1,3,6 month were 0.47, 0.93,0.53 There was no significant difference. The preoperative intraocular pressure obtained from preoperative to 1,3,6 month follow-up we found significant difference on 1,3, 6 month ( P=0,00),(P=0,00),(P=0,00) In the antiglaucomatous treatment the mean�Â�±SD preoperative up to 6 month was 3.08�Â�±0.87 to 2.60�Â�±0.94. Being significant at 6 month (p=0.016). We didnt found any significant difference in IOP,BCVA y Postop Drugs between both group. Complete success rate was 26.6 % and qualified success of 93.7%.

Conclusions:

Baerveldt 250 or Baerveldt 350 device outcome are similar. They permit to control de IOP , reduce de antiglacomatous drugs, mantein the BCVA without any significant difference between then.
