Combined cataract and glaucoma surgery in an aniridia patient: case report

Purpose:

To present the case of an aniridia patient referred to the Ophthalmology Department for ocular hypertension and cataracts.

Setting:

Ophthalmology Department, Hospital S�Ã�£o Jo�Ã�£o, Porto, Portugal.

Methods:

Results:

A 32-year old man was referred to our department for ocular hypertension and cataracts. There was a familiar history of aniridia. On his first observation visual acuity was assessed: count fingers in the right eye (RE) and hand motion in the left eye (LE). Pendular nystagmus, aniridia and corticonuclear cataracts were observed. Intraocular pressure (IOP) for both eyes was 60mmHg. Despite one month of maximum medical treatment IOP persisted over 30mmHg. The patient consented on combined cataract surgery with Ahmed valve (FP7) implantation to the LE. IOP was stable (10mmHg) with topical timolol and dorzolamide every twelve hours.

Conclusions:

Aniridia may occur with partial or complete iris hypoplasia. It frequently associates with foveal hypoplasia causing low vision and nystagmus. Aniridia patients may also present with cataracts, glaucoma and corneal opacification or neovascularization. This case reports the evolution of an aniridia patient and one treatment option. Genetic testing was requested and is the patient is being provided with genetic counseling. He maintains regular follow up visits at the department and is scheduled to undergo RE cataract surgery.
