Surgical and refractive outcomes of combined phaco-ExPress versus phaco-ExPress with partial deep sclerectomy: 24 month follow-up

Purpose:
To examine surgical and refractive outcomes of phaco-ExPress (P-ExPress) versus phaco-ExPress with simultaneously partial deep sclerectomy (PDS-ExPress) in open angle glaucoma treatment.
Setting:
The randomized prospective study included 114 eyes, of which 42 were subject to phacoemulsification combined with ExPress device implantation, while in 72 eyes phaco-ExPress device implantation with partial deep sclerectomy was performed. The participants were hospitalized in Ophthalmology Department of the Military Institute of Medicine in Warsaw, Poland.
Methods:
Examination of the patient included: measurement of visual acuity (BCVA), aplanation tonometry (IOP) and slit lamp examination. Some perioperative and postoperative complications and interventions (bleb needling, 5-FU injections, suturolisis) were analyzed as well. Patients examination was performed before the surgery and then on 7 and 30 day and on 1,3,6,12 and 24 months after the operation. Statistical methods like Mann- Whitney test and Wilcoxon test were used in this research.
Results:
There were no differences between the groups in visual outcomes (p>0.05).There were differences between the groups in the level of IOP. In the 1st, 3rd, 9th and 24th month after surgery IOP was statistically lower in theP DS-Express (P = 0.029, P = 0.049, P = 0.001, P = 0.023).
There were no significant statistical differences in the amount of medication between the groups before surgery, 24 months after surgery the amount of drugs was lower in the PDS-Express (P = 0.002).
The were no differences in number of postsurgery complications between the groups.
Conclusions:
PDS- ExPress has greater efficacy than the P-Express and has a similar safety profile.
