Purpose:
To evaluate the preliminary results of gonioscopy-assisted suture trabeculotomy (GAST) technique in reducing intraocular pressure (IOP) in surgical treatment of open-angle glaucoma
Setting:
Beyo��lu Eye Training and Research Hospital
Methods:
Twenty eyes of 19 open-angle glaucoma patients who could not be controlled with maximum medical therapy underwent GAST by the same surgeon.Nasal angle was displayed with Swan-Jacop goniolens.Temporal paracentesis was performed.The microsurgical forceps are used to guide the tip of 6/0 prolene suture through the 1-2 mm goniotomy cleft into Schlemm canal and to advance the tip around 360�Â�°.After the suture has coursed through the entire canal,the tip appears on the opposite side of the goniotomy cleft. The leading tip is grasped with the microforceps and pulled to the center of the anterior chamber.Nine patients were combined with phacoemulsification
Results:
The mean age of the fifteen male and four female patients was 60.7(29-79) Thirteen of the cases were PAG,6eyes were PEX,1eye was pigmentary glaucoma.The mean preoperative IOP was 21.7�Â�±7.1mmHg and the number of glaucoma medication was 3.9�Â�±0.6.Postoperative mean IOP and number of medication were 14.2�Â�±6.6 and1.3�Â�±1.5;12,3�Â�± 4.4mmHg and 1.8�Â�±1.5;11,1�Â�± 3.6mmHg and 2.0�Â�±1.5 and 11.3�Â�±2.8mmHg and1.7�Â�±1.5 in1st,3rd,6th and 12th months,respectively.The reductions of IOP and number of medication were statistically significant(p <0.05).No statistically significant difference in terms of IOP was found between GAST and combined surgical procedures(p=0.46).Additional filtration surgery was required in 2 cases
Conclusions:
Gonioscopy assisted suture trabeculotomy is a promising technique as an effective treatment for IOP reduction in open-angle glaucoma.
