Efficacy of femtolaser-assisted cataract surgery

Abstract Details

Purpose:

analysis of efficacy of femtolaser-assisted surgery of cataract

Setting:

Dnepropetrovsk regional clinical ophthalmic hospital, Dnepr, Ukraine

Methods:

340 eyes of 310 patients with senile cataract were included. 186 eyes (group 1) underwent femto-phaco using the Victus (TPV) and subsequent phacoemulsification or aspiration only (C-MICS) with pulsed ultrasound energy (Stellaris Visual Enhancement system, Bausch⺉�). Second group included 154 eyes in whom conventional C-MICS was perfomed, using pulsed ultrasound energy with the same machine (Stellaris). Patients in both groups were matched for age and nuclear density (LOCS III). Preoperatively and postoperatively uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), endothelial cell density (specular microscopy EM-3000), macular thickness (OCT) were evaluated.

Results:

Ultrasound parameters in the 1st group AVEUS were not more than 20%, ATU (absolute time of ultrasound) �â�€�“ 3,23�Â�±0,51s (p�â�‰�¤0,05), �Ð�•�Ð�¢U (effective time of ultrasound) �â�€�“ 0,15�Â�±0,08s (p�â�‰�¤0,05). In the 2nd group AVEUS made up 30% - 40%, �Ð��Ð�¢U �â�€�“ 6,62�Â�±3,25s (p�â�‰�¤0,05), �Ð�•�Ð�¢U �â�€�“ 3,05�Â�±2,8 (p�â�‰�¤0,05).

Conclusions:

1. Stage of femtolaser usage in surgery of cataract makes it possible to reduce dose of the ultrasound by 50% and more and to reduce timing of surgery. 2. In 100% of cases there is achieved ideally round central capsulorrhexis with overlapping of IOL edge by 0,5 mm; this provides a stable and aligned position of IOL, this being most important in implantation of toric, multifocal and accommodation IOL. 3. One of the advantages of femtosecond phacoemulsification is proportion of loss of endothelia cells, which made up 6,4%; this testifies to safety of the method.
