Femtosecond laser-assisted capsulotomy for the treatment of bilateral anterior capsule contraction: case report

Abstract Details

Purpose:

To describe a new technique for the treatment of anterior capsule contraction syndrome assisted by femtosecond laser.

Setting:

Oftalvist Group, Juan Bravo 1, Madrid, Spain

Methods:

A 51-year old woman developed severe fibrosis and occlusion of the visual axis in one eye maintaining a pinhole aperture on the fellow eye, two months after uneventful cataract surgery. Femtosecond laser capsulotomy with a fluid-filled interface system was used to perform a simultaneous anterior capsulotomy in both eyes. The energy level had to be raised from one eye to the following (4 �Î�¼J to 6 �Î�¼J) due to severe tags of the capsulotomy that had to be cut with scissors. Despite the increase in energy, it was not possible to achieve a free-floating capsulotomy either on the second eye.

Results:

Capsulotomy showed tags all around it in both eyes. Using forceps, it was possible to remove most of them by pulling from the border in the same way it is done for a manual capsulorhexis. Scissors were required to cut a very fibrous adherence on the right eye. One month after surgery, UCVA was 20/20 in both eyes and refraction returned to the early postoperative values. No marks on the IOL were reported after the capsule was removed in any of the eyes.

Conclusions:

Despite the drawbacks encountered with the femtosecond laser to treat anterior capsule contraction, our results suggest that this technology makes the procedure easier and safer than previous techniques described in literature.
