Efficacy and safety of femtosecond laser-assisted biaxial microincision cataract surgery compared with standard phacoemulsification cataract surgery: 12 months follow-up

Abstract Details

Purpose:

To evaluate the efficacy and safety of 1.4mm bimanual microincision cataract surgery (B-MICS) assisted by femtosecond laser LDV Z8 (FSL-BMICS) performed by an experienced surgeon comparing the results with B-MICS standard technique.

Setting:

Institute of Ophthalmology, University of Modena & Reggio Emilia, Modena, Italy (Head: Prof. Gian Maria Cavallini).

Methods:

This prospective, observational, comparative case series include 140 eyes who underwent B-MICS assisted by femtosecond laser (Group-A, 70 eyes) and B-MICS standard technique (Group-B, 70 eyes). Patients with any ocular pathology that could have affected visual outcomes during the follow-up period were excluded. Best Corrected Visual Acuity (BCVA), Surgically Induced Astigmatism (SIA), central corneal thickness (CCT), endothelial cell count (ECC) were performed pre-op and at 7 days, 1 month, 6 months, 12 months. Effective phacoemulsification time (EPT), balanced salt solution usage (BSS), total surgical time and complications during and after surgery were analyzed.

Results:

For Group-A we found FSL-related minor intraoperative complications in 18 cases(22.5%), post-operative complications in 3 cases(3.75%). For Group-B no complications. Mean EPT(sec.) was 5.05�Â�±3.32 in Group-A, 3.81�Â�±1.18 in Group-B. Mean Total Surgical Time(min.) 18.03�Â�±4.16 in Group-A, 12.62�Â�±4.37 in Group-B. Mean BCVA improvement(LogMar) at 6 months was 0.399�Â�±0.252 in group-A(p<.05) while it was 0.448�Â�±0.232 in group-B(p<.05); no difference between the two groups(p>.05). There was no statistically SIA neither CCT changes in both groups during the follow-up(p>.05); we found statistically significant mean loss of ECC:199.29�Â�±352.34cells/mm2 in Group-A, 250.60�Â�±387.32 cells/mm2 in Group-B at 3 months(p<.05) with no significative difference between the two groups(p>.05).

Conclusions:

FSL-BMICS with Z8 system is a safe and effective technique with a rapid visual recovery and a high patients�â�€�™ satisfaction compared with the standard technique. Thus the time-consuming adjustments in the early period, this technology helps the surgeons facilitating and standardizing the crucials steps of the surgery.
