Purpose:
To evaluate the use of femtosecond laser assisted arcuate incisions followed by topography guided ablation to treat post PKP astigmatism
Setting:
Dar El Oyoun Hospital, Cairo, Egypt
Methods:
5 cases are reported, 3 underwent femtosecond laser-assisted PKP 2 years earlier and 2 had manual PKP procedures 3 years earlier. All had postoperative astigmatism ranging from 4 to 10 D of cylinder. Stepped plan was carried out for all the cases where FS-Assisted Arcuate incisions using the Hanna nomogram was carried out then the residual cylinder was evaluated 6 weeks postoperatively, 4 cases underwent topography guided ablation for residual astigmatism.
Results:
All 5 eyes ended with less than 1 D of corneal astigmatism. UCVA was 20/30 in all eyes and BCVA was 20/20 in all 5 cases. Two of the cases had a Topography guided LASIK and the other two had topography guided PRK. The decision was based on the need for a shorter rehab. In the lasik group , 7mm femto flaps were created to avoid the edge of the graft. One case did not need ablation after the FS-assisted arcuate incisions.
Conclusions:
Combining FS-Assisted arcuate incisions and yopography guided ablation is safe and effective in the treatment of postkeratoplasty astigmatism
