Corneal collagen cross-linking for progressive keratoconus: one year follow-up

Abstract Details

Purpose:

To evaluate the safety and effectiveness of corneal collagen crosslinking (CXL) in cases of progressive keratoconus after one year.

Setting:

Bristol Eye Hospital, United Kingdom.

Methods:

From January 2015 - December 2015 (inclusive): 67 eyes of 64 patients with documented progression of keratoconus underwent epithelium off CXL treatment. Pre-operatively and post-operatively, uncorrected visual acuity, best corrected visual acuity, subjective refraction and corneal tomography (Pentacam) were performed. 20% alcohol was applied for 35 seconds then the corneal epithelium was manually debrided. The cornea was soaked with Riboflavin Rapid (Vibex) over 10 minutes and an Avedro cross-linking device was used to perform a continuous wave CXL procedure (9mW/cm2 over 10 min). Topical levofloxacin four times daily and dexamethasone 0.1% four times daily was given for 4 weeks post-operatively.

Results:

To date, 41 patients (44 eyes) attended post-operative follow-up at 12 months (range 10 to 17 months). The mean age of patients was 24 years (range 13 to 45). 8 patients were female and 33 patients were male. A change in Kmax of less than �.50 D from baseline at 12 months was defined as an index of non-progressive disease. 41 of 44 eyes (93.18 %) did not progress further. 3 of 38 eyes (7.89%) lost 2 or more lines of best spectacle corrected visual acuity.

Conclusions:

Epithelium off CXL is a safe and effective procedure in halting the progression of keratoconus. This study reinforces the safety and efficacy of this procedure.
