Keratoconus treatment: comparison of selected methods

Abstract Details

Purpose:

The purpose of this study was to compare efficacy and safety of selected methods for keratoconus treatment.

Setting:

The study was conducted in Ophthalmology Clinic, Centre of Postgraduate Medical Education and Eye Microsurgery Centre LASER, Warsaw

Methods:

120 patients age 18-53 treated with inttracorneal rings (INTACS) and intracorneal rings combined with cross-linking (INTACSॲ�). Patients were divided into 2 groups according to treatment method and keratoconus stage (Amsler's classification): 1. INTACS (n=60, stage 3), 2. INTACSॲ� (n=60, stage 3). Uncorrected and best corrected visual acuity (UVA, BCVA), maximal keratometry (Kmax), astigmatism (Ast), minimal pachymetry (CT) were evaluated before treatment and up to 36 months after. Efficacy, safety and complications were also evaluated.

Results:

Improvement of UVA (INTACS �,30 �0,14; INTACXॲ� 0,14 �0,12) BCVA (INTACS �,5 �0,17; INTACSॲ� �,2 �0,16), Kmax (INTACS -9,80 �4,48 D; INTACSॲ� -3,8 �2,04 D), Ast (INTACS -4,40 �1,84 D; INTACSॲ� -2,80 �0,79 D) was observed during 36 months in both groups. CT was stable in group INTACSॲ� whereas group INTACS presented progression.

Conclusions:

Both methods were safe and effective. Patients with advanced keratoconus can be treated with INTACS or INTACSॲ�. Cross-linking combined with intracorneal rings has additional advantage which is inhibition of corneal thinning (halted progression). Vast majority of complications and side effects were transient. Severe complications were observed in few patients.
