A pilot study to evaluate corneal volume as a determinant of effectiveness of cross-linking

Abstract Details

Purpose:

Biomechanical parameters have been shown to be the final determinant in progression of keratoconus. However,defining biomechanical parameters of the keratoconic cornea after cross linking (CXL) is not very accurate. The purpose of the present study is to evaluate the efficacy of CXL in halting progression by measuring corneal volume.

Setting:

Priyamvada Birla Aravind Eye Hospital, Kolkata, India

Methods:

16 eyes of 16 patients,3 females and 13 males, were evaluated just before, 3 months after and 6 months after CXL.. The refraction, keratometry values, Kmax, pachymetry value, corneal volume were recorded pre CXL and post CXL at 3 and 6 months interval.One way Anova test was used for statistical comparison between different groups.

Results:

Out of 16 eyes, 2 eyes of 2 patients showed increase in cylinder and Kmax, with a marked reduction in corneal volume after cross linking, at 6 months. In all other cases corneal volume stayed the same or decreased by a minimal amount. One way ANOVA showed no statistically significant difference in volume when comparing pre CXL and 3 months post CXL, pre CXL and 6 months post CXL or 3 months post CXL and 6 months post CXL.

Conclusions:

Corneal volume remains stable after CXL in keratoconus patients. We propose that corneal volume may be used as a parameter to monitor progression after CXL.
