Abstract Details
Purpose:
The aim of this study is to describe a technique for performing deep lamellar keratoplasty using femtosecond laser in the surgery of keratoconus: Intra Stromal Keratoplasty (ISK).
Setting:
Department of Ophthalmology, Hospital of Legnago, Verona
Methods:
A 31-year-old-male presented with keratoconus was referred for progressive corneal ectasia and worsening visual-acuity.His best-corrected-visual-acuity(BCVA) was20/100with corneal central thickness of390Âµm.He underwent femtolaser-assisted(Victus,B&L)DALK.The femtolaser with-real-time-OCT was used to create a vertical side cut on donor-cornea of310Âµm with anterior diameter of8mm and a posterior of8.1mm in order to generate a small step in the-external-profile.In the recipient-cornea was created a femtolaser-corneal-pocket of9.3mm of diameter and210Âµm of thickness. Two corneal-incisions of3mm at40Â°and220Â°were created in the recipient cornea, followed by manual dissection of the stromal pocket to allow the insertion of the donor stromal strip. Two stitches were positioned on the corneal-incisions.
Results:
The patient was followed up on day 1, day 5, at 2 weeks, 4 weeks. At each follow-up visit, complete ocular examination were performed. The cornea was clear and the stromal strip in the right position. At 4 weeks the BCVA was 20/40. No complications were encountered.
Conclusions:
ISK allows to perform a more safety surgical technique in keratoconus. It allow a fast recovery of the visual acuity without corneal suturing complications.
