4 years clinical outcomes of big bubble deep anterior lamellar keratoplasty in patients with keratoconus

Abstract Details
Purpose:
To report long-term clinical and refractive outcomes of big bubble deep anterior lamellar keratoplasty (BB-DALK) in patients with sever keratoconus.
Setting:
Magrabi Eye Hospital, Jeddah, Saudi Arabia
Methods:
Total of 89 eyes of 89 consecutive patients with advanced KC. They had a planned DALK surgery with big bubble technique. All cases were performed by a single surgeon (AD). Sutures removal at 12 months was planned with postoperative follow-up time equal to or greater than 4 years.
Results:
Mean postoperative follow-up was 76.9�Â�±23.2 months. Preoperative UDVA was 1.37�Â�±0.49, CDVA 0.84�Â�±0.56. MRSE -5.52�Â�±5.95 diopters, mean average corneal power (ACP) 54.6�Â�±6.7 D, topographic astigmatism 7.6�Â�±3.9 D. At 1 year while sutures still in-place, UDVA improved to 0.57�Â�±0.39, CDVA to 0.15�Â�±0.14, MRSE to -1.56�Â�±3.33 D, ACP to 44.0�Â�±2.0 D, and topographic astigmatism to 4.1�Â�±2.0D. At 18 months (6 months after ROS) UDVA was 0.64�Â�±0.54 and CDVA 0.12�Â�±0.11. Mean RMS decreased from 3.8�Â�±1.7 �Î�¼m to 1.1�Â�±0.5 �Î�¼m. No was eye converted to PKP.Stromal rejection was in 4.4%.
Conclusions:
DALK with the big bubble procedure was reproducible and effective for patients with moderate and advanced KC providing stable long-term visual and refractive outcomes.
