Aberrations induced by Descemet�â�€�™s membrane endothelial keratoplasty (DMEK)

Abstract Details

Purpose:

Knowledge of spherical aberration is important in assessing visual performance and in selecting intraocular lenses. The objective of the study is to evaluate the changes in spherical aberration and refraction in eyes after Descemet�â�€�™s Membrane Endothelial Keratoplasty (DMEK).

Setting:

The University of Ottawa Eye Institute

Methods:

This prospective study includes pseudophakic patients undergoing DMEK for indications of Fuchs Endothelial Dystrophy (FED) or Pseudophakic Bullous Keratopathy (PBK). Patients with any other ocular pathologies were excluded from the study. Ethics approval was obtained from The Ottawa Hospital Research Ethics Board. Preoperative, operative and postoperative data are collected at 1 week, 3 month and 6 month follow-ups. Outcomes assessed include best-corrected visual acuity (BCVA), uncorrected visual acuity (UCVA), intraocular pressure, refraction and spherical aberration. Aberrometry was performed using two different aberrometers: iTrace�â�„�¢and Pentacam. Spherical aberration and refraction will be compared preoperatively and at various follow-ups.

Results:

Preliminary results included 14 eyes undergoing DMEK, with indications including FED (n=13) and PBK (n=1). 6 patients were male, and the age ranged from 58 to 81 years. Compared to preoperatively, BCVA improved significantly at three month follow up (0.32�Â�±0.24 vs. 0.08�Â�±0.10 logMAR, p<0.05). Spherical aberration decreased at 3 month follow up compared to preoperative measurements (0.11 �Â�± 0.07 preoperatively vs. 0.09 �Â�± 0.05 �Î�¼m using Pentacam and pupil diameter 6.0 mm; 0.007 �Â�± 0.004 vs. 0.004 �Â�± 0.004 �Î�¼m using iTrace�â�„�¢ and pupil diameter of 2.0 mm).

Conclusions:

The study shows that spherical aberration is decreased after DMEK at 3 month follow up. Longer follow up is required to determine refractive shift and changes in spherical aberration
