Clinical outcomes of implantation of Ferrara intrastromal corneal ring segments in patients with astigmatism after penetrating keratoplasty

Purpose:
To evaluate the clinical outcomes of implantation of Ferrara intrastromal corneal ring segments (ICRS) in patients with astigmatism after penetrating keratoplasty (PKP).
Setting:
Hospital de Braga
Methods:
Retrospective consecutive case series of post-PKP patients who had ICRS implantation from January 2014 to September 2016, with manual mechanical technique. The following parameters were studied: uncorrected (UDVA) and corrected distance visual acuity (CDVA), keratometry (K) values, sphere and refractive astigmatism, corneal topographic astigmatism, minimum K, and maximum K.
Results:
The study evaluated 9 eyes of 9 patients. The mean UCVA improved from 0,055�Â�±0,05(range 0,01-0,1) to 0,2 �Â�± 0,13(range 0,05-0,4)(p<0,05) and the CDVA improved from 0,29�Â�±0,21(range 0,2-0,6) to 0,56�Â�±0,31(range 0,1-1,0)(p<0,05). The mean refractive cylinder was -4,52�Â�±3,41 diopters(D) preoperatively and -1,45�Â�±2.53D postoperatively(p<0,05). No patient lost visual acuity. The mean corneal topographic astigmatism decreased from 6,08�Â�±1,83D preoperatively to 4,50�Â�±2,20D postoperatively(p<0,05). The mean flattest K value(at 3,00 mm pupil) remained stable and the mean steepest K value decreased from 44,04�Â�±2.54 D to 41,40�Â�±2.63D(p<0,05). The maximum keratometry values also decreased from 52,81�Â�±18,21 to 50,04�Â�±17,27D(p=0,110).
Conclusions:
Intrastromal corneal ring segments effectively reduce corneal cylinder in patients with astigmatism after PKP, improving the irregular astigmatism, the visual acuity and the anisometropia. The procedure is safe and offers the rehabilitation of the binocular visual function.
