Results of penetrating keratoplasty in patients with complicated leukoma

Abstract Details
Purpose:
To assess the efficacy of penetrating keratoplasty in patients with severe corneal leukoma in 1 year after surgery.

Methods:
32 operations of penetrating corneal transplantation were made. Patients had ectatic leukoma with far-advanced stages of endothelial-epithelial dystrophy, extensive traumatic adhesive vascularized scars, perforated corneal ulcers, leukoma after keratitis against a background of autoimmune diseases. Visual acuity before surgery was from the correct light projection to 0.01. In 18.75% of cases the total keratoplasty with simultaneous Ahmed drainage implantation was performed. In 59.4% of cases keratoplasty was combined with cataract extraction, in 65.6% - with extensive reconstructive interventions.
Results:
Transparent engraftment in 1 year after surgery was noted in 83.3% patients with leukoma after system keratitis and in 75.0% - with traumatic scar lesions. In purulent perforated ulcers, a transparent engraftment of the transplant was noted in 60.0% of patients. In patients with severe corneal edematous degeneration, the graft retained transparency only in 58.3% of cases, after total keratoplasty - in 16.6%. The visual functions after surgery ranged from 0.02 to 0.2 (in the decimal system).
Conclusions:
Penetrating keratoplasty in complicated leukoma allows rehabilitating patients with severe corneal pathology in most cases and even in fatal situations allows preserving residual visual functions. The effectiveness of surgery directly depends on the severity of the initial state of the eye.
