Urgent penetrating keratoplasty as visual and tectonic graft for treatment of infective keratitis

Abstract Details

Purpose:

To evaluate urgent penetrating keratoplasty (PKP) to eradicate infectious process , remove the main bulk of infection and restore anatomical and fuctional outcome in treatment of aggressive infective keratitis not responding to early aggressive medical treatment

Setting:

MEMORIAL INSTITUTE FOR OPHTHALMIC RESAERCH Egypt

Methods:

Our study were done on 7 eyes with aggressive infective keratitis due to different causes they recieved full medical treatment with no respose and continue progression ,therapeutic urgent PKP were done and followed up for 8-12 months .All patients were done by same method and in one of them the lens was removed .

Results:

Clear graft were obtained postoperatively in 6 out of 7 eyes , late recurrence ocuur in one case after 2 months and respond to medical treatment , progression occur in one case . Best corrected visual acuity in 4 cases were between 0.2 and 0.5 , in 2 eyes was 0.01 due to cataract and deterioration occur in one eye .

Conclusions:

Our study shows that urgent therapeutic PKP for aggressive infective keratitis is successful treatment in eradicating the infection , restoring anatomical integrity of the eye and visual rehabilitation in most eyes . Early therapeutic PKP offers better results and visual outcome
