Planned Descemetorhexis without endothelial keratoplasty in eyes with Fuchs' corneal endothelial dystrophy

Abstract Details

Purpose:

The objective of this work was to describe the clinical course in Fuchs dystrophy where the central Descemet membrane was removed and not replaced with donor tissue.

Setting:

Rothshild Foundation, Paris, France

Methods:

6 eyes of 4 patients with Fuchs endothelial dystrophy associated with cataract had a 4mm intraoperative OCT guided descemetorhexis without endothelial keratoplasty simultaneously with phacoemulsification of the lens and IOL implantation.

Results:

In all cases, endothelial cells repopulated the majority of the descemetorhexis area within about 5 months. Pre-operative best corrected visual acuity (BCVA) was 0.6 � 0.2 LogMar. All patients reached a 20/20 (0 LogMar) post operative BCVA at 5 months. Central pachymetry measured with an anterior segment OCT decreased from 605 � 18 microns to 536 � 29 microns.

Conclusions:

Endothelial Keratoplasty might be delayed in early central Fuch dystrophy with moderate corneal edema and a careful descmetorhexis might represent an alternative treatment
