UT-DSEK with Russian femto-laser system: first experience

Abstract Details

Purpose:

To share our first experience with ultra-thin DSEK performed with Russian femto-laser system.

Setting:

S. Fyodorov Eye Microsurgery Complex Federal State Institution, Moscow, Russia.

Methods:

A technique of ultra-thin donor corneal tissue preparation from the endothelial side with �â�€�œVizum�â�€� femto-laser system (Optosystems, Troitsk, Russian Federation) had been developed. Femto-DSEK procedures with 4,5 mm temporal incision were performed for 20 patients: 12 with Fuch�â�€�™s dystrophy and 8 with secondary endothelial dystrophy. Observation period amounted 3 months for 15 patients and 12 months for 5 patients. UCVA, BSCVA, postoperative astigmatism, EC loss, central graft thickness and transplant-recipient complex thickness was evaluated.

Results:

In early postoperative period, no transplant dislocation was observed and 0 re-bubbling procedures were required. In 1 month after surgery, transparent engraftment was observed in 18 cases. In 2 cases, re-keratoplasty was performed due to primary graft failure. At 3 months, all 20 corneas were clear, EC loss amounted 24�Â�±5%. Maximum achieved UCVA was 0,6, BSCVA - 0,7, postoperative astigmatism - 1,25�Â�±0,25 D. Central graft thickness was 76�Â�±4 um and transplant-recipient complex thickness was 564�Â�±12 um. At 1-year follow-up all transplants were clear. Maximum BSCVA was still 0,7. EC loss amounted 37�Â�±8%.

Conclusions:

A technique of ultra-thin donor corneal tissue preparation from the endothelial side with �â�€�œVizum�â�€� femto-laser system allowed to produce predictable by thickness isomorphic transplants relatively safe for the endothelium and to achieve quite high visual outcomes with DSEK.
