Abstract Details
Purpose:
To study the results of penetrating autokeratoplasty in elderly patients with cornea dystrophy on one eye and blindness on the paired eye.
Setting:
St.Petersburg Branch of S.N. Fyodorov IR&TC ��Eye Microsurgery��, Russia.
Methods:
Autokeratoplasties have been performed in 14 patients with secondary endothelial-epithelial cornea dystrophy on one eye and blindness on the paired eye. The age of patients varied from 65 to 85. In 11 cases the clear cornea from a blind eye was used to replace an opaque cornea on a fellow eye, and the abnormal cornea was transplanted on the blind eye. In 3 eyes it was done just after enucleation of the blind eye. Mean follow up was 8 years.
Results:
Intraoperative complications have not been observed. During postoperative period graft rejection has not been recorded. In the long-term follow-up the mean endothelium cell loss was 3%, the mean visual acuity was 0.16�Â�±0.08.
Conclusions:
Bilateral penetrating autokeratoplasty is an effective and safe alternative to other techniques in selected cases. Visual outcomes may not be as good as after a routine keratoplasty, but it can eliminate the risk of graft rejection, graft infection and associated glaucoma due to long-term use of topical corticosteroids.
