Functional and anatomical success following intrastromal corneal ring segment implantation

Abstract Details

Purpose:

To evaluate anatomical and functional success following KeraRings (Mediphacos, Brazil) implanted in a University teaching hospital, over a three year period.

Setting:

The Royal Hallamshire Hospital, Sheffield, UK

Methods:

Retrospective review of patient case notes who received KeraRings implantation for corneal ectasia between July 2013 and Nov 2016. 78 patients received KeraRing insertion between these dates; case notes were available for 47 (61.5%) patients, (53 eyes). Uncorrected (UCVA) and best corrected visual acuity (BCVA) were compared pre and post operatively. Steep K (K2) and refraction measurements were recorded pre and post operatively and used as measures of corneal curvature.

Results:

53 eyes (23 Right, 30 Left). Mean preoperative UCVA of 0.92 logMAR (�Â�±0.47) improved to logMAR 0.50 (�Â�±0.37) one month post operatively (p = 0.000002). Mean preoperative BCVA of 0.36 logMAR (�Â�±0.36) improved to 0.20 (�Â�±0.24) one month post operatively (p = 0.009443). Mean preoperative positive cylinder was 6.00 D (�Â�±2.52), flattening to 3.13 D (�Â�±2.42) at one month follow up. For myopes, mean spherical equivalent preoperative -3.23 D (�Â�±4.41) and one month post operation was -2.37 D (�Â�±2.35). Mean first post operative K2 was 49.74 D (�Â�±4.53) and mean last K2 was 49.86 D (�Â�±3.71).

Conclusions:

There was a statistically significant improvement in both UCVA and BCVA following KeraRing implantation. Degree of irregular astigmatism improved and shows long-term stability, when comparing post operative K2 for one month follow up and final follow up (average 35 months) the K2 readings are similar (p=0.92). This study shows both a functional and anatomical improvement following KeraRing implantation in a patient population with corneal ectasia.
