Visual function and high order aberrations in corneal transplantation techniques

Purpose:
To investigate, describe and compare the contribution ocular higher-order aberrations of the anterior, posterior corneal surfaces and total cornea on visual function of eyes that underwent Descemet`s membrane endothelial keratoplasty (DMEK), Descemet Stripping with Automated Endothelial Keratoplasty (DSAEK), deep anterior lamellar keratoplasty (DALK) and penetrating keratoplasty (PK).

La Mancha Centro General Hospital - Alcazar de San Juan, Ciudad Real - Spain
Methods:
A retrospective comparison of consecutive eyes which underwent: DMEK 48 eyes, DSAEK 67 eyes, 50 eyes which underwent DALK, and 87 PK, and 57 uncomplicated eyes of 57 control patients. The keratoplasties were realized by a single surgeon.
In postoperative annual, and second year examination, each patient was analyzed with Pentacam (Pentacam HR, Oculus, Wetzlar Germany). The high order aberrations were calculated using the Pentacam software for Zernike analysis based in height. The central 4 and 6 mm area up to sixth Zernike order were analyzed.
Outcome data were compared between groups.
Results:
All keratoplasty groups had more aberration than normal population. Control eyes had significantly lower total high order aberrations of the anterior and posterior surface, and total cornea than the other groups.
We found an inverse correlation between the high order aberration and the visual acuity in the eyes underwent any keratoplasty.
Conclusions:
Evaluation of the corneal high order aberrations is helpful to understand the quality vision in the several keratoplasty techniques.
The high order aberration decrement the acuity visual in eye treated with corneal transplantation.
