Clinical outcomes of triple procedure pre-Descemet's endothelial keratoplasty (PDEK) and cataract surgery

Abstract Details

Purpose:

To evaluate clinical outcomes in patients after combined PDEK and cataract surgery.

Setting:

University Hospital Kralovske Vinohrady, Prague, Czech Republic

Methods:

20 eyes of 19 patients were enrolled in a nonrandomized retrospective single center study. In all cases we preceded combined PDEK and cataract surgery in one single procedure. The follow up time was 12,3 months (SD ± 3,1). The main indication was endothelial corneal dystrophy and presence of cataract. Following parameters were evaluated: UCVA, BCVA, spherical equivalent (SE), endothelial cell count at month 3, 6 and 12, rebubbling rate and other postoperative complications.

Results:

Mean UCVA was 0,62 (± 0,23 SD) 3month, 0,7 (± 0,25 SD) 6month, 0,76 (± 0,19 SD) 12month postoperatively. Mean BCVA was 0,76 (± 0,19 SD) at 3month, 0,84 (± 0,16 SD) at 6month, 0,83 (± 0,18 SD) at 12month follow up. Mean postoperative SE was -0,20D (± 0,9 SD) at month 3 and 6 and decreased to -0,11(± 0,68 SD) 12 months postoperatively. Mean endothelial cell density decreased by 38% at 1 year. Twelve eyes (60%) underwent rebubbling. In two patients (10%) antiglaucomatous treatment was started during postoperative period. No graft failed to clear. There was no graft rejection.

Conclusions:

PDEK combined with cataract surgery proved to be safe and effective method in patients with endothelial dystrophy and cataract. It shows stabile and very good results of visual acuity and satisfactory endothelial cell density of the graft.
