Purpose:
To evaluate the treatment outcomes (safety and efficacy) of manual superficial keratectomy, phototherapeutic keratectomy (PTK) and subsequent off-lable use of intra-operative application of mitomycin C (MMC) for Salzmann nodular degeneration (SND).

Methods:
This retrospective study included patients diagnosed with Salzmann��s nodular
degeneration without ocular co-mobidities. Pre- and postoperative examination included
autorefraction and subjective refraction for distance-corrected visual acuity (DCVA), slit lamp biomicroscopy, digital photography, corneal topography and anterior segment OCT. Epithelial removal was achieved 20% alcohol assisted and lamellar superficial dissection of Salzmann�â��s nodules was then performed. A 193-nm Ar-F excimer laser (Allegretto 200; WaveLight AG, Erlangen, Germany) was used for PTK with 7mm optical zone and 15�Î�¼m ablation. Mitomycin C 0.02% was applied afterwards for 30 seconds. All patients were followed-up for at least 6 months.
Results:
In total 50 eyes of 44 patients were included. Treatment of SND led to a significant increase of DCVA by average of four reading lines (far distance) up to ten reading lines (p<0.001), as well as dramatic reduction of hyperopia (myopic shift). Furthermore during follow-up, corneal topography showed regularization of astigmatism and all corneas appeared clear on slit lamp examination.
Conclusions:
This study confirms that the combined treatment including manual superficial keratectomy, PTK, and adjunctive MMC led to a significant and consistent improvement of visual acuity and quality in treated eyes.
