Outcome of the first six Boston Type I keratoprosthesis surgeries in Iraq

Abstract Details

Purpose:

Boston type I keratoprothesis (Kpro) is becoming the procedure of choice after multiple failed penetrating keratoplasties (PKP). Here we report the outcome of the first six Boston type I Kpro performed in Iraq.

Setting:

NIga Eye Center (NEC), Erbil, Iraq

Methods:

Six eyes of six patients underwent Boston type I Kpro with titanium back plate due to multiple failed corneal transplants by one surgeon (DA). The outcome measures are best-corrected visual acuity, Kpro survival and postoperative complications.

Results:

Mean postoperative follow-up was one year. Best-corrected visual acuity was finger counting at face preoperatively and 0.2 decimal postoperativelyl (p = 0.01). All the patients had multiple failed PKPs except for one who had primary Kpro but had failed PKP in the other eye. Two patients had previous Ahmed valve implanted while the other patients didn�â�€�™t have valve. No complications such as melt, extrusion, retro-prosthetic membrane, endophthalmitis, progression of glaucoma have been documented in all patients until now except for one patient who developed melt after his contact lens fell out from his eye. This was successfully treated later on.

Conclusions:

Boston type I Kpro could be a viable option after multiple failed PKPs or as primary procedure. This is the first six Boston Type I Kpro surgeries reported from Iraq.
