Descemet's membrane endothelial keratoplasty (DMEK): intraoperative and postoperative complications with clinical results

Abstract Details

Purpose:

The aim of this study is to evaluate the intraoperative and postoperative complications and clinical results of the consecutive descemet membrane endothelial keratoplasty (DMEK) surgeries in the subjects with endothelial failure without stromal opacification.

Setting:

A hundred eyes of 100 patients who underwent DMEK procedure with at least 6 months of follow-up at ophthalmology clinic of Adana Numune Training and Research Hospital between February 2014 and September 2016 were included in this retrospective study.

Methods:

Corneal transperancy, DM graft attachment and central corneal thickness (CCT) at preoperative and postoperative 1.day, 1.month and last follow-up visits, the unfolding time of DM graft, intraoperative and postoperative complications were recorded and analyzed.

Results:

During the surgery of the patient with a history of pars plana vitrectomy (PPV) with inferior iridectomy and fluid as a tamponade, drop of the DM graft was observed through the iridectomy space. DMEK combined vitreoretinal surgery for the drop of intraocular lens (IOL) with secondary IOL implantation was performed in a case (1%). At the end of the follow-up time, 85.7% of patients corneas were found to be clear, 2% was semi-clear and 12.2% were edematous.

Conclusions:

The intraoperative and postoperative complications were increased in patients with coexistence of another ocular pathology or complicated endothelial disfunction and during the learning curves of surgeons in DMEK procedure.
