Symptoms and signs of dry eye in patients with keratoconus

Purpose:
To evaluate symptoms and signs of dry eye in patients with keratoconus (KC) compared with healthy patients.
Setting:

Methods:
A total of 120 patients with KC and 87 aged-matched healthy subjects were enrolled in this prospective study. All eyes were examined with a rotating Scheimplug camera (OCULOS - Pentacam) to assess KC severity. A subanalisis by stages (Amsler-Krumeich classification) was done selecting the worst eye. The subjective symptoms of dry eye were evaluated with a self-administred questionnaire of the Ocular Surface Disease Index (OSDI, Allergan, Inc Irvine, California). Risk factors and signs of dry eye (fluorescein tear breakup time, Schirmer test without anesthesia and ocular surface staining determinated by Oxford Scheme) were evaluated in both groups.
Results:
KC patients had significantly higher OSDI scores (33.4 �Â�±25.1) compared with controls group (14.4�Â�±17,0) (p<0.01, independent sample t-test). The mean of the tear breakup time was 8.8�Â�±5.3 seconds and in patients with kearoconus and 11.2�Â�±7.4 seconds in the control group (p<0,01 independant sample t-test). The value of the Schirmer test was 16.9�Â�±10.4 mm/5 min and 18.9�Â�±9.9 mm/5 min in keratoconus and control group respectively (p=0.143, t-test). Ocular surface staining was observed in 38 keratoconus subjects (31.7%) compared with 6 control subjects (6.9%) (p<0.01, Chi-square test).
Conclusions:
The parameters evaluated in our study demostrate that patients with keratoconus had a greater average score in mean value of the OSDI than the control group. This study also shows that patients with keratoconus presented shorter tear breakup time and greater staining of the ocular surface than control subjects.
