Keratoplasty in keratoconus: impact of atopy

Abstract Details
Purpose:
To study the progression of atopic patients who underwent a keratoplasty for keratoconus compared with non-atopic patients.
Setting:
Ophtalmology department, Mohammed VI University Hospital, Marrakesh, Morocco
Methods:
Patients were divided into two groups: a group A with associated atopic manifestations (asthma, allergic rhinitis, atopic dermatitis and ocular allergy) (N = 13) and a second group B with no atopic manifestations (N = 29).
We compared the two groups for those variables: Age at diagnosis, Stage of Keratoconus, Best corrected visual acuity before transplant, Hydrops occurrence, Age at the graft, complications after transplantation, and better corrected visual acuity after keratoplasty.
Results:
The mean age at diagnosis of keratoconus was 21 years in group A versus 25 years in group B. Keratoconus was classified as bilateral stage IV in 95% in group A and 78% in group B. The best corrected visual acuity was less than 1 logMAR in 78% (group A) and 50% (group B)
After transplantation:
- In group A: rejection (1 eye), cataract (2eyes), hypertonia (2eyes)
- In group B: rejection (2 eyes), cataract (3 eyes), hypertonia (2 eyes)
The best visual acuity corrected after transplantation was greater than 0.3 logMAR in 61% against 65% (respectively A and B)
Conclusions:
Studies on a bigger sample could help to better understand the role of atopy in the genesis, expression and evolution of corneal transplant complications on keratoconus
